摘要
目的:探讨血管性血友病因子(vWF)在2型糖尿病微血管及大血管并发症中的意义。方法:采用双抗夹心ELISA法检测20例正常人(对照组)和48例2型糖尿病患者的血浆vWF,其中糖尿病微量白蛋白(MAU)组26例,糖尿病合并大血管病变组22例。结果:糖尿病微量白蛋白尿组及大血管病变组,血浆vWF水平均高于正常对照组(P<0.01),且大血管病变组高于微量白蛋白尿组(P<0.05)。经相关分析,血浆vWF水平与糖化血红蛋白(HbA1 c)、尿白蛋白分泌率(UAER)呈正相关,而与低密度脂蛋白(LDL)、胆固醇(CHOL)及血压无相关。结论:内皮损伤是2型糖尿病血管并发症发生、发展的重要机制,vWF可能是血管并发症早期临床敏感指标之一。
Objective: To investigate the clinical significance of von willebrand factor(vWF) alterations in type 2 diabetes mellitus patients complicated by microangiopathies and abnormalities of large blood vessels. Methods : Measured the plasma levels of vWF antigen in 20 normal controls and 48 patients with type2 diabetes mellitus (26 patients suffering from microangiopathies with microalbuminuria and 22 patients suffering from abnormalities of large blood vessels ) using a sandwich enzyme-linked immunosobent assay (ELISA). Results: The plasma vWF levels in patients were significantly higher than that of normal controls ( P 〈 0.05 ). Compared with patients with microalbuminuria, the patiens with large blood vessel damages had an even higher plasma vWF levels. Moreover, the plasma vWF levels were positively associated with gtycosylated hemoglobin( HbA1 c) urinary albumin excretion rate (UAER) but did not associated with low density lipid (LDL), cholesterol(CHOL) and blood pressure based on correlation analysis. Conclusion: Endothelium damage was responsible for the occurrence and progression of abnormalities of blood vessels, and ptasma vWF level could be used as a sensitive parameter for predicting the blood vessel complications in patients with type 2 diabetes mellitus .
