摘要
目的阐明补体C3、C3c和C3d分子在IgA肾病肾组织中的表达,探讨C3、C3c和C3d在IgA肾病发病中的作用。方法对36例IgA肾病和43例无IgA沉积的系膜增生性肾炎患者进行回顾性分析,对其肾组织进行抗人C3、C3c和C3d抗体检查,在免疫荧光显微镜下观察C3、C3c和C3d在肾组织上各自的表达。结果补体C3在IgA肾病组肾小球上的表达高于系膜增生性肾炎组患者。C3c和C3d仅在IgA肾病上发现表达;它们的共同表达与肾脏病理损害有关(r=0.99176,P=0.0009)。结论补体C3、C3c和C3d在IgA肾病肾组织上高表达,而且C3c和C3d共同表达与肾脏损害有关,可能是向终末性肾衰竭转化的标志。
Objective To study the molecular characteristic of deposit of C3 complement and its breakdown components, to investigate the role of C3 and its breakdown components in pathogenesis of IgA nephropathy(IgAN). Methods According to clinical and pathological diagnosis,79 cases of renal biopsy were classfied into IgAN group, and mesangial proliferative glomerulonephritis group without IgA deposite. Their renal frozen sections were incubated by anti-human C3 , C3c, C3d antibody respectively. Immunofluorescene microscope was used to evaluate the deposits of C3 ,C3c,C3d. Result The C3 deposits were more higher in IgAN group than those in non-IgA deposite group ,however,the C3c and/or C3d deposits were only expressed in the IgAN group, moreover, a high percentage with positve C3c and C3d deposits is highly correlated with the severity of renal pathological injury( Pearson r =0. 99176,P =0. 0009). Conclusion C3c and C3 d are expressed in the primary IgA nephropathy, moreover, expression of both C3c and C3 d is correlated with renal pathological injury. C3 complement and its breakdown components play an important role in the pathogenesis of IgA nephropathy.
