颌下腺移位术预防放疗后口干燥症的动物实验模型 被引量:4

摘要 目的:通过建立兔颌下腺移位的动物实验模型,探讨兔颌下腺移位术的可行性。方法:实验组:7只新西兰兔均将左侧颌下腺移位至颏下区。术后接受不同剂量(2~14Gy)的电离辐射,移位侧的颌下腺在照射野之外。对照组:7只新西兰兔将左侧颌下腺移位至颏下区,术后不接受电离辐射。20周时将移位侧及未移位侧的颌下腺游离之后全部取出,观察其病理变化。结果:实验组未移位侧颌下腺组织病理变化:在照射后20周时,照射剂量6、8Gy的颌下腺组织中腺泡细胞间隙增宽、腺泡明显萎缩、腺泡数目明显减少,间质纤维组织增生、增厚;照射剂量10、12、14Gy的颌下腺组织腺泡的间隙明显增宽。实验组和对照组移位侧颌下腺组织病理变化与正常颌下腺的组织学形态无差异。结论:颌下腺移位术通过将颌下腺移位至颏下区,避免了直接照射,成功地保护了移位的颌下腺。 OBJECTIVE:To investigate the feasibility of submandibular gland transfer in preventing radiation-induced xerostomia in rabbits. METHODS: The left submandibular salivary gland was transferred to the submental space by operation in 7 New Zealand rabbits. Eight days after operation, the rabbits were given different doses (2-14 Gy) irradiation with the transferred one outside the irradiation scope. As the control group, the left submandibular gland was transferred to the suhmental space by operation in another 7 New Zealand rabbits that were not irradiated after the operation. Twenty weeks after the irradiation, the untransferred and transferred submandibular glands were removed to observe the late morphologic changes. RESULTS: The histopathology of the untransferred rabbit submandibular glands in the experimental group: Twenty weeks after the radiation, doses of 6 and 8 Gy caused wideness of some spaces between acni, decrease of seromucous acini, apparent atrophy of seromucous acini, the fibrous tissue thickened; 20 weeks after radiation, doses of 10-14 Gy causes apparent wideness of the spaces between acni, local necrosis of acini, apparent atrophy of acini, pronounced reduction of serous granules in the cell of serous ducts, necrotic cell segments in the ducts. The fibrous tissue increased and thickened, especially around the striated ducts. The histopathology of the transferred rabbit submandibular glands in the experimental group: No significant difference was found in any histologic parameter between the transferred (shielded) submandibular glands in the experimental group and the untransferred submandibular glands in the control group. CONCLUSION: The rabbit's submandibular gland transferres to the submental space is protected successfully by avoiding direct radiation.
