重度阻塞型睡眠呼吸暂停综合征患者冠心病高发病率机制的研究 被引量:10

摘要 目的检测血浆GMP-140、D-二聚体在重度阻塞型睡眠呼吸暂停综合征(OSAS)和冠心病(CHD)患者体内浓度的变化,探讨OSAS患者CHD发病率增高的机制。方法对重度OSAS组18例、CHD组22例、重度OSAS合并CHD组(伴组)12例患者和对照组20名进行多导睡眠图(PSG)监测,酶联免疫吸附法(ELISA)检测血浆中GMP-140与D-二聚体浓度。结果重度OSAS组、CHD组、伴组血浆GMP-140、D-二聚体浓度与正常对照组相比,差异有统计学意义(P<0.05),而三组之间差异无统计学意义(P>0.05)。GMP-140与D-二聚体浓度呈正相关,相关系数r=0.69,P<0.05。结论重度OSAS患者因乏氧体内存在血管内皮(包括冠脉内皮)损伤、血小板活化、凝血系统激活和继发性纤溶亢进,这些可能是OSAS患者CHD的发生、发展机制。 Objective To evaluate changes of plasma levels of GMP-140, D-dimer in patients with OSAS and CHD, and to discuss the cause of higher morbidity in patients with CHD who are OSAS diagnosed. Methods 18 severe OSAS patients, 22 CHD patients,12 combined OSAS and CHD patients, 20 controls were recruited for this study. Polsemnography (PSG) was administered to all subjects in sleep disorders center, GMP- 140 and D-dimer level in plasma were measured by in-house enzyme-linked immunosorbent assay(ELISA) technique. Results In OSAHS group, CHD group, combined OSAS and CHD group, plasma levels of GMP-140 were significant difference(P〈0.05). Three groups levels of D-dimer had no significant difference between them(P〉 0.05). GMP-140 and D-dimer were correlated significantly and positively (r=0.69,P〈0.05). Conclusion For lack of oxygen, patient with severe OSAHS, coronary artery endothelial damaged and occur activation of platelet,matter of coagulation system combined with fibrinolytie activation occur. They may be the reason which OSAHS will have CHD.
