摘要
目的研究靶控输注异丙酚复合吸入异氟醚在小儿腭裂修复术中应用对血流动力学的影响及苏醒期情况的比较。方法选择40例ASAⅠ级施行小儿腭裂修复术患者,随机分为靶控组(Ⅰ组)和恒速组(Ⅱ组),常规静脉麻醉诱导后行气管内插管,维持期闯机械通气;1组采用血浆靶控输注技术泵入异丙酚,同时复合吸入异氟醚。Ⅱ组采用速率为3mg·kg^-1·h^-1输入异丙酚,同时吸入异氟醚。分别于诱导前(T1)、插管后5min(T2)、手术开始时(T3)、术毕时(T4)、拔管后2min(T5)几个时点,记录血压(BP),心瘁(HR)、脉搏饱和度(SpO2),并与诱导前进行统计学比较,同时记录两组患者术毕停药自主呼吸恢复时同,呼之睁跟时间和拔管时间。结果与诱导前相比,两组在插管后5min(T2)的SBP、DBP下降明显(P〈0.05),在手术开始时Ⅱ组的HR较诱导前明显增快,在拔管后2min的HR变化Ⅰ组明显低于Ⅱ组,呼吸恢复时间、睁跟时同、拔管时间两组相比,Ⅰ组明显缩短(P〈0.01)。结论靶控输注异丙酚复合吸入异氟醚用手小儿腭裂修复术麻醉效果满意,对血流动力学影响轻微,苏醒迅速而平稳。
Objective To research the application of injection propfol by control injection of target complex inbreathing isofluran in infantile palatorrhaphy, it influences on blood stream dyn and the contrast of revival period. Methods 40 patients with ASA Ⅰ grade palatorraphy,were divide into two groups randomly, Ⅰ group was control injection of target and Ⅱ group was constant rapidity, Ⅰ group injected propfol by control injection of target at the same time complex inbreathing isoflurane. Ⅱ group injected propfol in constant rapidity of 3mg· kg^- 1· h^- 1 at the same time inbreathing isoflurane, The patents in two groups were noted BP, HR and SpO2 respectively in T1, T2, T3, T4 ,T5 and compared with pre-abduction, nodded the comeback time of freedom breath,the time of goggle and the time of evulsion cannulation in all the patients, Results Compared with pre-abduction ,SBP and DBP in T2 in all patients descended obviously(P 〈 0.05), beginning operation, the patient's HR expedited than the pre-abduction in Ⅱ group,evulsion cannulation 2min, FIR change of Ⅰ group was lower than Ⅱ group, the comeback time of freedom breath, the time of goggle and the time of evulsion cannulstion ,the time of Ⅰ group was shorten( P 〈 0.01 ). Conclusion Injection propfol by control injection of target complex inbreathing isofluran in infantile palatorrhaphy,the anaesthesia effect is satisfactory, the influence of blood stream dyn is slight and the revival is quick and balanced.
