后路椎间融合器加椎弓根系统治疗腰椎滑脱症 被引量:19

摘要 目的评价采用后路椎管减压、椎体间融合器(Cage)融合和椎弓根钉复位内固定系统治疗腰椎滑脱的疗效。方法对27例腰椎滑脱患者采用全椎板减压、Cage加节段性椎弓根钉复位内固定治疗。结果术后随访12~36个月,所有患者都获得了椎体间完全融合。术后1年及最终随访时JOA评分恢复与术前比较差异有显著性(P<0.01);并发症有脑脊液漏1例,1例足趾背伸功能减弱。结论后路椎管减压、Cage加节段性椎弓根钉系统复位内固定治疗腰椎滑脱,可增强稳定性,提高疗效。熟练掌握该项技术,可减少或避免相关并发症。 Objective To evaluate the clinical effect of lumbar spondylolisthesis treated by posterior decompression, interbody fusion cage and pedicle screws fixation devices. Methods 27 cases were treated with pedicle screw fixation and posterior lumbar interbody fusion cage after decompression. Results The follow-up ranged 12 ~ 36 months. All cases achieved bony fusion at final follow-up. Clinical status was evaluated by the recovery rate of Janpanese Orthopedic Association ( JOA ) score. It was significant different in JOA scores compared with pre-and post-operation ( P 〈 0. 01 ). There were 2 cases with complications:the dural tear in 1 case, and L5 nerve root injury in 1. Conclusions It can provide rigid fixation, high rate fusion and excellent clinical effect in the treatment of spodylolisthesis with cage and pedicle screw fixation. Surgery complications can be avoided by improving surgical technique.
