应用异体脱细胞真皮基质修复口腔黏膜缺损82例分析 被引量:8

摘要 目的:总结应用异体脱细胞真皮基质(acellulardermalmatrix,ADM)修复口腔黏膜缺损的疗效,分析影响愈合的因素。方法:采用ADM修复82例浅表口腔黏膜缺损,缺损部位包括唇、颊、舌、腭、咽,牙龈、口底和前庭沟,缺损面积最小1cm×2cm,最大4cm×6cm,平均8.96cm2。术后密切观察2周。结果:随访1个月~1a,成活情况为优者75例(91.46%),良7例(8.54%),无1例失败。收缩情况为优者52例(63.41%),良25例(30.49%),差5例(6.1%)。术区组织形态与正常组织相比无明显差异,黏膜活动度良好,患者无疼痛或异物感。结论:ADM具有组织相容性好、取材方便、适应性广等优点,可作为口腔黏膜缺损修复的理想材料;其愈合与缺损部位、缺损面积和固定方法有密切关系。 PURPOSE: To summarize the clinical effects of allograft acellular dermal matrix(ADM) in repairing defects of oral mucosa and to investigate the factors affecting healing. METHODS: Eighty-two eases with defects of the oral mucosa were repaired with ADMs. The sites of defects included the lip, buccal mucosa, tongue, palate, pharynx, gingiva, floor of mouth and vestibule. The sizes of defect were between 1cm×2cm and 4cm×6cm,the mean area was 8.96cm^2.All patients were observed closely for 2 weeks after operation. RESULTS: The patients were followed up from 1 month to 1 year. The survival rate was excellent in 75 eases (91.46%), good in 7 cases (8.54%) and no failure ease. The contracture rate was excellent in 52 cases(63.41%), good in 25 cases(30.49%) and poor in 5 cases (6.1%). There was no significant difference between the surgical sites and the normal oral mucosa. All patients got a better function recovery of oral mucosa without pain or discomfort. CONCLUSIONS: Allograft acellular dermal matrix may be considered a useful reconstructive option for patients with oral mucosal defects. The main advantages of ADM are good in biocompatibility, unlimited supply, available in various sizes, etc. The healing of ADM is related to the sides of defect, the sizes of defect and fixation methods.Supported by Medical Science and Technology Research Project of Foshan City(Grant No.2004-7).
