摘要
目的:探讨抑癌基因PTEN在喉癌组织中的表达与喉癌临床特征的关系,了解喉癌的生物学行为。并从细胞水平探讨喉癌手术安全切缘的范围。方法:用P-V9000免疫组织化学法检测38例喉磷癌组织及距癌组织边缘2 mm、5 mm1、0 mm的组织中PTEN蛋白的表达,并取10例声带息肉组织作对照。结果:PTEN在喉癌组织中阳性表达率为60.53%,在对照组中阳性表达率为100.00%,两组比较差异有显著意义(P<0.05)。在距癌旁2 mm、5 mm1、0 mm组织中PTEN阳性表达率分别为71.05%、89.47%、100.00%。PTEN阳性表达率依癌组织→癌旁2mm→癌旁5 mm→癌旁10 mm→声带息肉组织顺序递增,癌组织与癌旁5 mm、10 mm组织比较PTEN阳性率之间有显著性差异(P1<0.05,P2<0.05),而与癌旁2 mm处组织之间无统计学意义。结论:PTEN可以作为判断喉癌生物学特性的参考指标,PTEN的低表达在喉癌中发挥着重要的作用。喉癌手术切除范围以距肿瘤5mm作为安全切缘的标准较为适宜。
Objective:To investigate the relation between expression of PTEN in laryngeal carcinoma and its' clinical characteristics and to explore the biological action of laryngeal tumor and the range of safe surgical margin. Methods: Thirty-eight laryngeal squmous cell carcinoma(LSCC) specimens and different regions adjacent carcinoma (2 mm,5 mm, 10 mm) were examined for the expression level of PTEN protein by immunohistochemical method(PV9000). Ten polyps of vocal cord were used for controls. Results:The positive expression rate of PTEN was 60.53 % in laryngeal carcinoma tissues and 100.00% in control group with a statistical difference. The rate were 71.05,89.47%, 100.00% ,respectively in larynx adjacent mucosa of were 2 mm,5 mm, 10 mm away from the cancers. The results also showed that the expression of PTEN proteins increased as tissue progressed in sequence from carcinoma tissues to 2 mm,5 mm, 10 mm distant mucosa adjacent carcinoma and polyp of vocal cord tissues. The statistical differdnce between carcinoma tissues and edge group(5 mm.10 mm)was significant(P1〈0.05.P2〈0.05) respectively,but it was not significant between carcinoma tissues and tissues near the tumor. Conclusion:PTEN may act as a reference index in judging the biological speciality of LSCC. Lower PTEN may be associate with LSCC. It is appropriate to regard 5 mm away from tumors as resection margins for surgical treatment of laryngeal carcinoma.
