三种剂量舒芬太尼对冠状动脉旁路移植术患者血液动力学及血浆儿茶酚胺的影响

摘要 目的:比较三种不同剂量舒芬太尼对冠状动脉旁路移植术(coronary artery bypass grafting,CABG)患者血液动力学、血浆儿茶酚胺浓度和术后恢复情况的影响。方法:将18例CABG患者随机分为3组,麻醉中分别应用舒芬太尼总量0.8μg/kg,2.0μg/kg和3.2μg/kg,记录不同时点的血液动力学指标并测定血浆肾上腺素和去甲肾上腺素浓度。结果:S组术后拔气管导管时间为(12±3)h,明显早于H组(16±4)h,M组(18±4)h(P<0.05),三组间的血液动力学指标C I,SVR和PVR比较差异无显著性,与基础值比较H,M组的NE浓度降低,而S组的NE浓度变化不大。结论:应用总量2.0μg/kg和3.2μg/kg舒芬太尼能够更好地抑制术中血浆NE浓度,而应用总量0.8μg/kg舒芬太尼可较早拔除气管导管。 Objective: To compare the effect of three different doses of sufentanil on hemodynamics and catecholamine and the condition from postoperative in patients undergoing coronary artery bypass grafting. Methods:Eighteen patients undergoing coronary artery bypass grafting were randomized to three groups. The dose of sufentanil is 0.8 μg/kg,2.0μg/kg and 3.2μg/kg respectively. The index during different period of time about hemodynamics and the adrenalin and noradrenalin were determined. Results: Compared the time of preserving trachea postoperative, The S group (12 ± 3 )h is shorter in evidence than H group (16 ± 4)h and M group [ (18± 4) hi (P 〈 0. 05). Compared with the index of hemodynamics including CI, SVR and PVR, there is no marked difference. Compared with the basic value, The concentration of noradrenalin in H and M group is reduced, but there is no change in the S group. Concluion:Deliver the total dose of sufentanil 2.0 μg/kg and 3.2μg/kg could restrain the concentration of noradrenalin, 0. 8 μg/kg of sufentanil could reduce the time of extubation.
