摘要
目的探讨TNF-α及TNF-RⅠ在椎间盘突出致坐骨神经痛发病机制中的作用。方法采用自体髓核移植至L4、5神经节,建立根性神经痛的动物模型,应用VonFrey针丝及RTY-Ⅰ型热痛测仪对神经行为的改变进行检测,采用免疫组化法检测TNF-α及TNF-RⅠ在神经节及神经根中的表达并分析其与神经行为改变之间的相关性。结果自体髓核移植可以导致明显的机械刺激痛觉过敏现象,而无明显的热刺激痛觉过敏,TNF-α及TNF-RⅠ在自体髓核移植术后4天表达开始增强(P<0.05),术后1周表达最强(P<0.01),术后3周逐渐恢复正常,与痛觉过敏的改变对时间有显著相关性。结论髓核组织触发了内源性TNF-α的释放,其与TNF-RⅠ一起参予了椎间盘源性坐骨神经痛的发生,可能是引起根性神经痛的关键细胞因子,应用抗TNF-α的药物可能对治疗椎间盘源性坐骨神经痛有一定的作用。
Objective To investigate the role of endogenetic TNF-α in the mechanism of sciatica due to disc herniation. Methods Autologous nucleus pulposus was applied to the peripheral of the L4 and L5 nerve roots and DRGs. Based on this animal model,Von Frey filaments and a thermal testing device(RTX-1 type)was used to quantify behavior test. Immunocytochemistry was used to find out the expression changes of TNF-α ,TNFR-Ⅰ . Results The prolein expression of TNF-α, TNFR-Ⅰ increased significantly (P〈0.05)in DRGs,nerve roots at postoperative 4d,especially at postoperative week 1(P〈0.01). At postoperative week 3,the expression restored gradually to normal. There was a time correlation between the protein expression of TNF-α and the changes of neuro- behavior. Conclusion Endogenetic TNF-α was involved the mechanism of mechanical allodynia after autologous nucleus pulposus implanted. Applying medication.of anit-TNF-α may become an effective treatment of sciatica due to disc herniation.
