摘要
报告20例胸腹联合伤的诊治经验。其中闭合伤11例,穿透伤9例,20例中合并伤多达32处,伤情均较严重,伴出血性休克者4例,中毒性休克者3例。治疗后19例康复,1例因胃绞窄坏死行全胃切除术及空肠-食管吻合术,术后死于中毒性休克。作者针对膈肌伤易于漏诊之原因,结合本组病例的诊治经验,提出了可疑膈肌伤的临床表现。对手术径路的选择及术中注意事项进行了讨论。
The diagnosis and treatment Of 20 cases of penetrating and blunt diaphragmatic injury are dealt with in this paper. Of all the 20 cases, there were 11 closed wounds, and 9 penetrating wounds. There were 32 associated injuries in this group. The condition of the wound was severe, and 4 cases were combined with hemorrha-gic shock, 3 with septic shock. Nineteen patients recovered after treatment and one patient died of septic shock after total gastrectomy and esophago-jejunogastrosto-mosis for gastric strangulation and necrosis.In connection of the misdiagnostic causes of diaphragmatic injury, and in the light of our own therapeutic experience, we put forward the clinical manifestations of dubious diaphragmatic injury ,Selection of surgical approaches and matter, to be considered in operation and discussed.
