血液流变学改变在兔病理性氧供依赖性中的作用

摘要 作者在家兔内毒素致病理性氧供依赖性(POSD)模型上,考察氧供(Do2),氧耗量(Vo2)及血液流变学变化特点,初步探讨血液流变学改变在POSD中的作用。结果显示,在Do2-Vo2的关系中,对照组依赖段的斜率与内毒素组斜率相差显著(P<0.05).POSD时血浆粘度、纤维蛋白原浓度、红细胞聚集指数、血沉等增加,肺、骨骼肌、肾脏有较多微血栓形成,提示有弥漫性血管内凝血发生.血液粘滞性、红细胞聚集性增加,红细胞变形性降低,对微血管内血流产生严重干扰,导致组织血液灌流量不足,气体和物质交换受阻.这些改变则可能是外周组织及器官缺血甚至衰竭的原因之一. n the rabbit model of the pathological oxygen supply dependence(POSD)induced by endotoxin,the change features in oxygen delivery(Do2),oxygen uptake(Vo2)and hemorheology were observed and theeffect of the changes on microcirculation during POSD was studied. The results show that the slopes,i.e.the oxygen extraction rates(ER),in the“dependent”segment of the control and endotoxin groups were 0.73and 0.47 respectively,which were significantly different(P<0.05).During POSD,there were increases inthe plasma viscosity,the concentration of fibrinogen , the erythrocyte aggregation index and Sedimentationrate,and many microthrombi in the lung,kidney and skeleton muscles,this suggests that the disseminatedintravascular coagulation(DIC)occurred.The increases in blood viscosity,aggregation of erythrocyte anddecrease in deformability of erythrocyte disturbed the blood flow in the microvessels,induced deficiency of in-fusion in microcirculation and blocked the exchange of gases and substances.These may be the reason of is-chemia even failure in peripheral tissue and organ.
