246例骨关节镜手术预防性使用抗菌药物调查及经济学分析 被引量:1

摘要 目的:调查骨关节镜手术使用抗菌药物预防术后感染的合理性,分析选择不同抗菌药物对患者住院总费用的影响。方法:从2003年全部出院病例中选取行骨关节镜手术的246例病例,对患者一般情况、抗菌药物使用情况和药物经济学指标进行调查分析。结果:调查表明,100%的患者预防性使用了抗菌药物,其中206例(83.7%)使用了一种抗菌药物,40例(16.3%)合并使用了2种以上的抗菌药物。246名患者共使用了24种抗菌药物277例次,使用最多的药物分别为力深、凯德林和利君派舒。患者平均用药时间5.26d,抗菌药物平均费用1384.49元,抗菌药物费用占患者住院总费用的比例为15.42%。选择不同的抗菌药物对患者住院总费用有明显影响。结论:预防性使用抗菌药物已经成为骨关节镜手术的常规措施,但在使用中普遍存在着明显的不合理现象。选择适当的抗菌药物和疗程可以有效降低患者的治疗费用。 Objective: To investigate the proper administration of antimicrobial agents in arthoscopic surgery with aim to effectively prevent surgical site infection.Methods :A review of 246 patients undergoing knee arthroscopic operation from January 2003 to December 2003 was carried out. The expense of antimicrobial agents and its influence on patient's total surgical cost was also observed.Results: Antimicrobial prophylaxis was performed in 100% of the 246 cases with the use of antibiotic drug combinations vs single antimicrobial agents (16.3% vs 83.7%, respectively) involving 24 kinds of antimicrobial agents used 277 times in all patients, in which Clindamycin Phosphate was used most frequently and Ampicillin+sulbactam, Cefoperazone + sulbactam followed. None was infected after operation and had serious adverse drug reaction. The cost of consumption of antimicrobial agents per case was 1384.49 yuans (15.42% of total surgical cost). The average therapeutic course was 5.26 days (1-20days).Concl,,sions: It seemed that antimicrobial prophylaxis had been a routine prescription in arthroscopy operation, and unreasonable drug utilization was observed in the majority of cases. The medical employees should receive better training of infection prevention. Some measures should be taken to improve the level of rational drug utilization and to reduce the expense of antimicrobial agents.
